rpm ordering failures

I'm starting to see this now:

Fetched 3407kB in 1s (2731kB/s)
error: rpmtsOrder failed, 3 elements remain
E: Ordering failed for 3 packages

The above is with RPM::Order "true" set.  I get the same thing without
it set.

[root at ares ~]# rpm -q apt
apt-0.5.15lorg3.1-5787avx

Any idea what the problem is?  I was trying to do a full version upgrade
of Annvix last night with apt and couldn't because of this (although
there were 22 packages it noted).  This is with a simple handful of
packages to upgrade:

[root at ares ~]# apt-get dist-upgrade
Reading Package Lists... Done
Building Dependency Tree... Done
Calculating Upgrade... Done
The following packages will be upgraded
  chkconfig dcron diffutils initscripts iproute2 iputils less lib64mhash2 lib64usb0.1_4 lib64utempter0 mailx makedev mingetty

The following NEW packages will be installed:
  ethtool
17 upgraded, 1 newly installed, 0 removed and 0 not upgraded.
Need to get 0B/3407kB of archives.
After unpacking 343kB of additional disk space will be used.

I don't *think* it's anything I did in terms of packaging... urpmi is
handling all of this flawlessly.
