[patch] off_t

Hi,

Another (not so harmless) off_t-patch, furthermore gradually approaching
the dark sides of libapt's internals.

Remark: This patch is the result of eliminating a couple of dubious
typecasts in apt-pkg/acquire-item.cc, and back-tracking them into other
files.
